Last updated: 7 July 2026

Please note: CLM Community Support is an independent, non-registered NDIS provider. We support self-managed and plan-managed NDIS participants and privately funded and self-managed aged care clients. We are not affiliated with, or endorsed by, the National Disability Insurance Agency (NDIA).

Our commitment to your privacy

CLM Community Support (“we”, “us”, “our”) is committed to protecting your privacy and handling your personal and health information responsibly. Because we provide health and support services, we are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs), regardless of our size or turnover. This policy explains what information we collect, why we collect it, how we use and protect it, and how you can access or correct it.

What information we collect

Depending on your relationship with us, we may collect:

  • Personal information — such as your name, address, phone number, email, date of birth and emergency contacts.
  • Sensitive information — including health and medical information, disability and care needs, medications, NDIS or aged care plan details, and funding arrangements. Sensitive information is only collected with your consent and where it is reasonably necessary to provide your care.
  • Website information — when you use this website we may collect standard technical data (such as your IP address, browser type and pages visited) through cookies and analytics tools to help us improve the site.

How we collect information

We collect information directly from you wherever possible — for example through our enquiry and contact forms, phone calls, emails, meetings and assessments. With your consent, we may also collect information from third parties involved in your care, such as your GP, hospital, pharmacy, allied health professionals, family members, carers, or your plan manager or support coordinator.

Why we collect and how we use your information

We use your information to:

  • assess your needs and provide, coordinate and review your care and support;
  • communicate with you, your carers and other health professionals involved in your care;
  • manage bookings, rostering, invoicing and payments;
  • meet our professional, insurance and legal obligations; and
  • respond to your enquiries and improve our services.

Disclosure of your information

We do not sell your information. We only disclose it where necessary to provide your care or where required or authorised by law — for example to GPs, pharmacists, hospitals and allied health providers involved in your care, to your NDIS plan manager or support coordinator, or to government bodies where legally required. Where we use third-party service providers (such as IT or website providers), we take reasonable steps to ensure they protect your information.

How we store and protect your information

We take reasonable steps to keep your information secure and to protect it from misuse, loss, unauthorised access, modification or disclosure. Records are stored securely and access is limited to team members who need it to provide your care. We retain information only for as long as required for care and to meet our legal obligations, after which it is securely destroyed or de-identified.

Accessing and correcting your information

You have the right to request access to the personal information we hold about you and to ask us to correct it if it is inaccurate, out of date or incomplete. To make a request, please contact us using the details below. We will respond within a reasonable time.

Cookies and website analytics

This website uses cookies and analytics tools (including Google tools) to understand how visitors use the site so we can improve it. You can disable cookies in your browser settings, though some parts of the site may not work as intended.

Complaints

If you have a concern about how we have handled your personal information, please contact us first and we will do our best to resolve it. If you are not satisfied with our response, you can contact the Office of the Australian Information Commissioner (OAIC) on 1300 363 992 or at oaic.gov.au.
