Hospital Discharge Rapid Response | Bundaberg & Wide Bay Clinical Transition

Rapid 24 to 48-Hour In-Home Clinical Transition for Hospital Patients in Bundaberg & Wide Bay

Are you a hospital social worker, discharge planner, clinical nurse coordinator, or family member preparing for a patient’s transition from hospital to home? Delayed discharges due to missing community nursing, complex personal care, or equipment setup create unnecessary stress, prolong acute hospital stays, and increase infection exposure risks.

CLM Community Support provides a dedicated Hospital Discharge Rapid Response Portal across Bundaberg, Bargara, Childers, Gin Gin, Burnett Heads, and Moore Park Beach. Led by Linda Miller, Registered Nurse, our clinical team mobilises within 24 to 48 hours to establish safe, hospital-grade in-home support for NDIS participants, Support at Home / Home Care Package clients, and private patients transitioning from Bundaberg Base Hospital (Wide Bay HHS) and Friendly Society Private Hospital (“The Friendlies”).

Why Clinical Discharge Planners Rely on CLM Community Support

  • 24–48 Hour Response SLA: We attend bedside clinical handovers or virtual case conferences and conduct comprehensive community risk assessments within 24 to 48 hours of referral.
  • AHPRA Registered Nurse Leadership: Linda Miller RN directly oversees clinical nursing handoffs, wound staging, medication profile reviews, and high-intensity care plans.
  • Bridging & Rapid Intake: We assist hospital social workers with rapid service agreements, emergency funding allocation, and coordination across NDIS Core/Capacity Building budgets and Aged Care Support at Home allocations.
  • Local Staff Continuity: Experienced local Bundaberg support workers trained in patient-specific transfer techniques, hoist safety, and infection prevention.
  • Zero Bed-Block Guarantee: We eliminate unnecessary bed-days by ensuring home-based support workers, clinical nursing visits, and equipment readiness are confirmed before the patient leaves the ward.
