Helping Eligible Community Members Access Fully Funded Wound Consumables

CLM Community Support is proud to support the Australian Government’s Chronic Wound Consumables Scheme (CWCS) — a national initiative that fully covers the cost of approved wound consumable products for eligible Australians living with diabetes and a chronic wound.

This includes essential items such as dressings, bandages, adhesives and other clinically endorsed products used to support healing and ongoing wound management.

What Is the Chronic Wound Consumables Scheme?

The CWCS is an Australian Government program designed to:

  • fully subsidise the cost of wound consumables for eligible patients

  • reduce financial pressure associated with chronic wound care

  • improve wound-healing outcomes and quality of life

  • support ongoing wound monitoring and management in the community

The scheme allows authorised health professionals—such as registered nurses, nurse practitioners, GPs, podiatrists and Aboriginal and Torres Strait Islander health practitioners—to order wound consumables through a secure online portal, with products delivered directly to the patient’s home.

Who Is Eligible?

According to the official program guidelines, the CWCS supports people who:
✔ have diabetes
✔ have a chronic wound (lasting longer than 30 days or not healing normally)
✔ meet one of the following age criteria:

  • 65 years and over, or

  • 50 years and over for First Nations people

Who Is Not Eligible?

The scheme cannot be used by people receiving wound care consumables through:

  • NDIS funding

  • Department of Veterans’ Affairs programs (including RPBS dressings)

  • Residential Aged Care, Home Care Packages, or Community Home Support Program

  • State-funded public wound clinics

  • Public or private hospitals (post-operative or outpatient care)

If you’re unsure about eligibility, our clinical team can guide you through the process.

How CLM Community Support Helps

Our registered nurses and support team can assist eligible clients through every step of the CWCS process:

1. Clinical Assessment

A trained and eligible health professional from our nursing team will assess chronic wounds, overall health needs, and suitability for the scheme.

2. Enrolment Into the Scheme

Once eligibility is confirmed, our nurse will enrol the patient into the CWCS using the online ordering system.

3. Ordering Fully Subsidised Consumables

We select from the approved CWCS wound consumables list and order everything needed to support wound healing—at no cost to the patient.

4. Home Delivery of Products

Consumables are shipped directly to the patient’s home or our clinical office for collection, ensuring consistent supply.

5. Ongoing Wound Care Support

CLM Community Support can provide:

  • regular wound checks and dressing changes

  • complex wound care and monitoring

  • holistic nursing care plans

  • education for clients and carers

  • daily living support around wound-related needs

Our services are person-centred, compassionate and designed to promote independence, safety and a better quality of life.

Why Wound Care Support Matters

Chronic wounds can take a significant toll on a person’s physical, emotional and financial wellbeing. They can:

  • take months or years to heal

  • cause pain, infection and reduced mobility

  • lead to repeat hospital visits

  • impact confidence and daily living

The scheme was created to reduce these barriers and ensure people with chronic wounds can access the care products they need to heal—when they need them.

CLM Community Support is dedicated to improving outcomes for clients across Bundaberg and the Wide Bay Region.
